In the case of an infection in the bone, how does the doctor replace the nail without the bone breaking? After all, the bone has not yet healed and it can break if there is nothing to hold it

My guess is he does not replace the nail til the infection is gone which will be normally 4-6 weeks. You will likely be on IV antibiotics like Vancomycin or Daptomycin and sometimes oral too for better coverage.

Could that infection get worse due to lifestyle eg. sleep position!
