Curiosity Question: When does Paley make you stop lengthening?

I have heard from some and via Dr. Paley himself that he will stop lengthening the patient if they are showing various signs of x y or z.

I am wondering, for anyone who has consulted with Paley or from CLL veterans, what are those limits?
