3 inches is fine with internals, it's a bit much with externals.

5 cm on tibia seems like the magic number

Imo, safest max gain would be 6 cm external tibiae, 6.5 cm internal femurs for 5 inches total, assuming you go to a reputable doctor. But like with any surgery there are risks, and the more surgeries you do the more risks you are taking.
