You should check whether the surgeon you're working with actually accepts credit card without charging a fee. I had one surgeon do the initial surgery and another surgeon do the removal. Both charged a 3% credit card fee. So you're not actually saving any money via the credit card rewards.
If you're in the USA, another thing to look into is Care Credit. It's basically a credit card specifically for cosmetic procedures that charges 0% APR for a year.
