Since it is an elective cosmetic surgery, is it eligible for FMLA? any insights will be greatly appreciated, thanks.

There is no easy answer. It depends on the person who will handle your case. Your company's HR, the way your doctor writes you your letter might affect this. You may also lose some privacy of getting this procedure done.
