At first I thought people lie in hospital bed the entire time of the lengthening, but then I read about living in hotel and walking in diaries. Can someone enlighten me on how this works? During the time, do you need a caretaker or hotel service? Will I be in a wheelchair or am I bed stricken? I'm thinking of renting a place nearby and getting a cleaner once a week and I can cook or buy food myself.

This forum has tons of diaries where people detail their lengthening and recovery.  I suggest you skim a few for whatever doctor(s) you're looking at.

In short, you don't NEED help but it would be significantly easier if you DO HAVE help. You get the break, you then are usually on restricted activity for 6 to 12 weeks depending on length desired. Then you rehab and rebuild. You won't be bedridden all the time but your pain will dictate how much rest you need etc.
